New patient registrations
We have an open list and welcome requests for registration from patients living in or moving to the practice area.
You need to complete BOTH a registration form and a new patient health questionnaire.
Registration forms
Use one of these forms
- Drs Mirza, Sukhani & Partners NHS online patient registration (recommended)
- Britannia House Surgery NHS online patient registration (recommended)
- You can also register by completing a form available from reception.
Once completed we shall contact you to verify your I.D. Ensure you have enough repeat medications from your previous surgery to last you a few weeks, as we are unable to issue any medications until a new patient medical has taken place with our GP.
You will have a named, accountable doctor who is responsible for coordinating your care. You can still talk to or make appointments to see any of our doctors or nurses, not just your named GP.
We recommend that new patients undertake a health check with a practice nurse.
